Richard Cote is absolutely right: the most important thing concerning your personal choices at the end of life you'd want to know about your doctor is, whether he is prepared to "help" (in the broadest sense possible) you whenever you ask for his "help" or not. You should ask this question (or rather talk with your doctor about this sensible issue) long before your life actually starts to end; this will give you enough time to find another when (s)he is not prepared (whether religious or not!).  I know of many Dutch religious doctors who do perform euthanasia!
